At Attuned Programmes Ireland, our Outreach Services are designed to provide consistent, compassionate, and person-centred care to individuals and families in their homes and communities. We work with children, young people, and adults who need additional support to thrive independently, stay connected to their communities, and build brighter futures. Each outreach programme is tailored to the specific needs of the individual or family and developed in collaboration with key professionals, including social workers and the HSE.
Our experienced Social Care team delivers a wide range of services, including daily home supports, family support, behavioural planning, supervised access visits, transport to school and appointments, and 24/7 on-call crisis assistance. By fostering trust, promoting independence, and encouraging integration, we help each person feel safe, empowered, and supported in achieving their personal goals.
